APPS of Louisiana Privacy Policy
APPS of Louisiana understands the privacy of information entrusted to
it by clients is vital, and APPS of Louisiana is committed to
protecting that privacy.
Protecting Information
APPS of Louisiana maintains security policies and
practices to protect appropriate information from unauthorized
access and inappropriate disclosure, alteration or destruction.
Security may include, but is not limited to, encryption, physical
access security and other appropriate technologies. APPS of Louisiana
continually reviews and enhances security systems as necessary.
Breaches by employees are subject to disciplinary process.
Personnel are required to execute an acknowledgment of their receipt
and understanding of APPS of Louisiana's policies and procedures
containing confidentiality requirements. APPS of Louisiana does
not provide private information to the general public. APPS of
Louisiana does not compile or distribute mailing lists or consumer
marketing data from information received from clients. APPS of
Louisiana does not modify or correct information obtained from other
persons or organizations, since such information is not prepared by
APPS of Louisiana.
Obtaining and Sharing Information
APPS of Louisiana obtains information ordered by its clients using
sources identified by clients for the purposes of their
underwriting and servicing of insurance policies, APPS of Louisiana
handles such information in accordance with its clients'
directions. APPS of Louisiana does not share information it
obtained for a client with other clients, unless the client has
consented. APPS of Louisiana is authorized by clients to act
as their representatives in obtaining information on individuals
including but not limited to the following:
- Health history / Medical information/ Prescription Medications
- Physicians in attendance, hospitalizations
- Laboratory and /or other studies
- Family history
- Use of tobacco
- Habits, use of alcohol and other drugs
- Driving records
Sources of such information may include some or all of the
following: individual interview(s), Blood and urine specimens,
physician exam, radiologic studies or other types of studies as
required by the companies underwriters.
Opting Out
The applicant need only decline to provide the requested
material. APPS of Louisiana only gathers and discloses the
information pursuant to the clients authorization.
